Flu Shots for Students

Health Hero America (HHA) will be providing flu shots to our students on October, 27th.

Parents will need to fill out forms and sign in order for the student to receive their flu shot on campus. Those who register and sign up will receive a phone call with more information on what to do and where to go on October 27th.

The attached flyer lists the different times and location parents can go to get the forms and receive help filling them out if needed. We will also have blank forms in the front office that parents can come by and get if they do not need assistance filling them up. Any forms that are filled out on their own need to be returned back to the front office by next Tuesday, the 20th.
